    The Nurse provides direct Veteran care and care coordination, as it pertains to Veterans and their care within the Home Based Primary Care Program (HBPC). The mission of the HBPC is to provide comprehensive, interdisciplinary, primary care in the homes of Veterans with complex medical, social, and behavioral conditions for whom routine clinic-based care is not effective. The HBPC target population is frail and/or chronically ill Veterans who require interdisciplinary health care teams, continuity and coordination of care, and the integration of diverse services to cover their complex medical, social, rehabilitative, and behavioral care needs. These Veterans need comprehensive, longitudinal home care services as they age to maximize function, minimize institutionalization, and maintain quality of life. The HBPC RN Case Manager will: Provide initial and continued assessment of nursing needs of the Veteran.
