Uses motivational interviewing and health coaching to guide patients in achieving goals of health promotion and disease prevention. Participates in panel management: Completes pre-clinic phone prevention up to 24 hours before scheduled appointment. Assists with chronic disease management and other care coordination needs. Use of telehealth technology to render care. Completes view alerts assigned within 48 hours. Provide New Patient Orientation when you encounter new patients.
