Former EDRP participants ineligible to apply. We are seeking a Supervisory Social Worker for the Housing and Urban Development-VA Supportive Housing Program to join our team at the H. John Heinz III Department of Veterans Affairs Medical Center. They collaborate closely with the Social Work and Health Care for Homeless Veterans Program (HCHV) administrative staff to assist with the necessary documentation and administrative responsibilities required by HUD and the various agencies and organizations involved in providing quality care to the program's recipients. Assist in planning, establishing, and implementing policies and procedures: developing individual or group goals and objectives as well as monitoring, operating, evaluating, coordinating, and overseeing the HUD-VASH staff to which they are assigned. Responsible for all performance measures for the HUD-VASH Program and reports monthly statistics to the National Office. They provide professional, clinical, and administrative management of their assigned HUD-VASH team in the Comprehensive Social Services Service Line under the umbrella of Healthcare for Homeless Veterans Program (HCHV) and community partners. HUD-VASH teams include multi-disciplinary staff, including nurses, social workers, and addiction specialist, who frequently travel between sites and spend approximately half of their time in the community. Ensure that ongoing, intensive reviews and appraisals are completed to ensure the accomplishment of objectives and adherence to standards set by governing bodies. They collect and submit HUD-VASH Program participant data as outlined by HOMES evaluation procedures on a regular basis as well as in compliance with the VA, VHAPHS, VISN, VACO and HUD documentation and reporting requirements. The supervisor formulates and implements protocols and policies for the program on an on-going basis to meet the ever-changing needs of the program's target population. Provide oversight of the direct clinical and case management services of veterans and/or families enrolled in the program. The program necessitates detailed coordination efforts with public housing authorities, property managers and landlords in the following states/counties: Pennsylvania - Allegheny, Westmoreland, Beaver, Washington, Fayette, & Greene. West Virginia - Hancock, Brooke, Ohio, and Marshall. Ohio - Jefferson, Belmont, and Monroe. Coordinates the screening and admission process for the HUD-VASH program. Uses advance clinical training, insight, and experience to interpret data and to identify viable treatment options, including assessment for eligibility and clinical appropriateness of a HUD-VASH admissions or subsequent discharge. Provides independent consultation and makes recommendations to the interdisciplinary team on the treatment plan. Routinely interviews and counsels Veterans to resolve various problems or conflicts, and assists the veteran in reaching treatment goals. A substantial portion of the workload directed by this position is carried out at public housing authorities, VAMCs, and locations that are physically removed from the main unit under conditions that make day-to-day supervision difficult to administer; frequent phone and e-mail contact as well as regular conference and video calls with supervisees and other relevant staff occurs to ensure the smooth continuum of care.     The general responsibilities of the Advanced Orthotist-Prosthetist are to administer, supervise, or perform work concerned with providing clinical and laboratory prosthetic and orthotic services to disabled patients at all VISN 4 designated facilities and their associated Community Based Outpatient Centers (CBOC) under the Supervision of Assistant Chief PSAS. This work requires a specialized knowledge of the medical and psychological problems directly related to the use of prosthetic devices; and a specialized knowledge and understanding of the fabrication and satisfactory fitting of prosthetic devices. Included but no limited too: Serving as an advisor to physicians, therapists and other medical professionals with regard to indications and contraindications of prosthetic and orthotic componentry, prescription criteria Authorizing the purchase, fabrication or repair of prosthetic devices. Maintaining continuous liaison with manufacturers and dealers of prosthetic supplies, inspecting their facilities and services and participating in the award of prosthetic appliance contracts; Participating in investigative/research project studies by selecting patients as pilot wearers of newly developed prosthetic items, evaluating the usefulness of such items and compiling of data; and Giving technical and administrative advice and assistance on prosthetic problems to other facilities. Assesses patient for indications/contra-indications of appropriate treatment protocols and discusses the assessment with the physician/requesting provider for purposes of developing the overall treatment plan. Considers new and emerging technologies in designing unique or innovative devices to accommodate or treat a wide range of complex medical conditions, which include unusual problems or complications. Considering the information developed through the assessment as well as time management workload considerations, designs custom devices using standard components to optimize patient performance for difficult and complex conditions and may include CAD/CAM systems. Selects materials and components to be used in fabrication of the device and determines appropriate equipment to be used in fabrication of the device. Fabricates components, or selects and modifies components, and assembles these into the finished device. Uses CAD/CAM system as appropriate in this process and performs preliminary alignment. Fits the device on the patient, assures preliminary alignment, and performs subsequent dynamic alignment. Assesses and/or modifies fit and function of the device as fabricated and makes appropriate modifications to insure optimum patient outcome. Evaluates patient's ability to don/doff the device provides training to the patient and/or caregiver in its use and maintenance. This may also include training the clinical staff. Explains procedures and processes to the patient to reduce anxiety and gain support and cooperation in all stages of the process. Maintains the patient's record, including the electronic record, in accordance with established JCAHO/ABC/BOC/Medical Center/VISN policies. Attends professional and manufacturer's training seminars.
